I received this letter approx. 10 hours ago:

Seeing as how I already completed it I am unable to return to it for you to view the screen shots I was intending on making. However, I do have another idea of getting in there. So tomorrow I might make some screen shots. Not promising anything, though.
For now, I can only give you a short summary. Basically there are 5 pages. By the way, obviously not everyone gets this form, so don't get your hopes up. In ea. page there are variety of questions which you must answer, or boxes which you must select appropriate to your desirable opinion, i.e.
"What should we do with the drop rate?" [ ]Increase [ ]Stay as is [ ]Decrease,
"How is it leveling up during level XX?" (x6 of these questions I believe there were, level 1-10, 11-20, 21-30, 31-40, 41-50, 51+) [ ]Easy [ ]Normal [ ]Hard [ ]Haven't played it yet,
"What do you think is the balance of the classes as it is now?"--#1;Magician, #2; Archer, #3; Acolyte, #4; etc. [ ]Too Strong, [ ]Moderate [ ]Too Weak, etc.
There's a bar in top center of the the screen which shows the % of your result so far; thus 5 pages ea. page is 20%.
I don't remember much but there were also some misc. questions like:
"How did you hear about SoS?" [ ]Search Engine [ ]IGN (or some famous gaming website) [ ]Friend [ ]Other; specify,
"Do you have any suggestions on how we can improve SoS?" (optional).
"How should the order of the classes be called?" {Rogue, Felon, Assassin, Shadow}; {*don't recall the first*, Rogue, Assassin, Shadow Master}, {Acolyte, Clergy, Priest, Bishop}; {Neophyte, Clerk, Priest, Paladin}, etc. Anyway, if you're lucky enough, by tomorrow I'll have screen shots. For now, you finally know how the form is bestowed upon the consumers and a relatively small summary of what it contains.
If you want to know whether or not there is a certain question there or something, feel free to ask, I may not remember the questions unless I'll hear what the question is and there's an 87% chance that'll I'll recall whether it was there or not.
