To Reproduce this bug these are the steps:
Play Team Mode with items enabled,
Hit all the arrows and just before u hit the last one get your friend to throw a dynamite, due to lag - it registers as you perfecting the bar, but force miss the new arrow that appears because of the dynamite item, and you will lose your arrow combo - but on your character it will continue your perfect combo for the next bar. I'll post a screen shot later on to illustrate what i mean.
Suggestion of Fix:
Don't allow items to be able to activate mid way through the bar, and only when the bar is about to start.
