How to get treasure chest from kq nowadays???

It's been a while since the last time I played fiesta. I tried Spider KQ few times, and didnt even get one treasure chest.
Whats the rule now:confused:
PS. I wasn't dead, my inventory wasnt full, and I survived. I really dont see why I cant get one..:(
