are any permanent? or just all the 30 day use? also is the 30 day use calculated by the hours you spend in game or does it expire even if you dont play?

there are a few that are permanent... the red, blue, and yellow menian sets, the wedding dress/tux (both versions) are as well... there have been times when other costume pieces, like the cat ears, have been offered as permanent as well (this may still be, i'm lazy to check)
but typically, costumes last 30 days.. they last 30 calendar days (whether you're in-game or not) from the moment u activate it, down to the hour (roughly, usually an hour or so more it seems). there are (was?) 7-day costumes as well... just read the descriptions before buying.

From time to time permanent costumes are re-released in the stores or you may obtain one during a special event, though they are very rare occasions.
As for the timer, unfortunately it goes down even if you are logged out.
