Does anyone know which ports to open on my router for this game to run smoothly?

I wouldn;t know. I have a Dlink router and have not had to alter the ports or NAT at all.
It may be that you have windows firewall running as well. It does this by default on Windows XP SP2.

I run a linksys router and haven't needed to open any ports. Like tomcat025 says, I would look at any software firewall you may be using on your machine.
