I died as the last mob was being killed.* It was my first death so I had more left.* Before I could click ok, the "Success" popped up.* I did not get a chest or exp.

Because you were still dead, it sucks I know but that is what happens. You have to be alive when the boss falls in order to receive the exp/chest.
I went through the same thing once or two. Very annoying.

My first TWO kq quest that managed to get a reward chest were EMPTY, the third had a single blessed elrue !
Not been able to click fast enough to get into another instance.
